چکیده
Published in January 2011, the aims of the Valvular Academic Research Consortium (VARC) were to arrive at a consensus: 1) on the most appropriate clinical endpoints reflecting device and patient effectiveness and safety; and 2) to standardize the definition of endpoints for valverelated clinical trials (2). This multidisciplinary endeavor brought together interventional cardiologists, cardiac surgeons, noninvasive cardiologists, regulatory bodies, professional societies, and industry with the hope to facilitate the reporting, communication, and evaluation of this new catheter-based therapy. The goal of VARC was not to establish “utopian-like” definitions but to arrive at a consensual agreement based on state-of-the-art practices and materials. In this issue of JACC: Cardiovascular Interventions, Hayashida et al. (3) investigated the incidence, predictors, and impact of VARC-related vascular complications in 127 patients undergoing transfemoral implantation with the

Endocarditis after transcatheter aortic valve implantation: a current assessment
Transcatheter aortic valve implantation (TAVI) constitutes an established treatment in inoperable or high perioperative risk patients with severe aortic stenosis, demonstrating similar mortality rates (at 30 days and 1 year) with surgical aortic valve replacement (SAVR). Various complications have been reported during TAVI, weeks or months post procedure. The most frequent causes of transcathet...
متن کاملImpact of Mean Platelet Volume on Combined Safety Endpoint and Vascular and Bleeding Complications following Percutaneous Transfemoral Transcatheter Aortic Valve Implantation
BACKGROUND Vascular and bleeding complications remain important complications in patients undergoing percutaneous transfemoral transcatheter aortic valve implantation (TF-TAVI). Platelets play an important role in bleeding events. Mean platelet volume (MPV) is an indicator of platelet activation. The objective of this study was to assess whether low MPV is an indicator of major vascular and ble...
متن کاملTranscatheter Aortic Valve Replacement: A Review Article
Transcatheter aortic valve replacement (TAVR) is a novel therapeutic intervention for the replacement of severely stenotic aortic valves in high-risk patients for standard surgical procedures. Since the initial PARTNER trial results, use of TAVR has been on the rise each year. New delivery methods and different valves have been developed and modified in order to promote the minimally invasive p...
متن کاملSevere intraprocedural complications after transcatheter aortic valve implantation: calling for a heart team approach.
OBJECTIVES Transcatheter aortic valve implantation (TAVI) has emerged rapidly. Despite unanimous recommendations and potentially fatal intraoperative complications, the heart-team approach is not comprehensively adopted by all centres. We sought to characterize severe intraprocedural complications during TAVI requiring immediate surgical or interventional bailout manoeuvres and evaluate outcome...
متن کاملLimitations and difficulties of echocardiographic short-axis assessment of paravalvular leakage after corevalve transcatheter aortic valve implantation
To make assessment of paravalvular aortic leakage (PVL) after transcatheter aortic valve implantation (TAVI) more uniform the second Valve Academic Research Consortium (VARC) recently updated the echocardiographic criteria for mild, moderate and severe PVL. In the VARC recommendation the assessment of the circumferential extent of PVL in the short-axis view is considered critical. In this paper...
